US‐Only Restricted Session

Attendees of the US‐Only Restricted Session must submit a Verification Form verifying US citizenship or permanent residency. Completed forms must be submitted by August 14, 2009 to:

Cancellation Policy

Refunds will be made to those registrants who cancel, less a $50 processing fee. A written cancellation request must be received by August 5, 2009. No refunds will be made after this date. Substitutions will be accepted through the time of on-site registration.

The Utah State University Research Foundation reserves the right to cancel this program due to insufficient enrollment and limits its liability to registration refunds only.

Student Rate Eligibility

The student registration rate applies to individuals who are enrolled as full-time undergraduate or graduate students. Those who are employed full-time and are taking courses are not eligible for this rate. If you have questions regarding your eligibility, please contact Stephanie Halton or Brittney Hunt.

Dietary Request

Food substitutions can be made for those with special dietary needs. If you or your guest has unique dietary requirements, please provide details on the registration form or attach a separate sheet. It is important that we know in advance (before the conference begins) in order to make the necessary menu adjustments.

Persons with Disabilities

Reasonable accommodation is available for persons with disabilities. If you have special needs, please provide the necessary information on the registration form or attach a separate sheet. Ten working days notice is needed to prepare materials and services.
